Abstract
Modern software systems increasingly fail in ways that are not attributable to bugs, outages, or incorrect execution. Instead, they fail semantically: they continue to operate while no longer reliably embodying their original intent. This paper examines a recurring structural pattern in software development in which meaning is distributed, implicit, and reconstructed locally, rather than captured explicitly and enforced normatively. We argue that existing tools—tests, specifications, type systems, reviews, and AI-assisted generation—operate downstream of meaning and therefore cannot prevent its erosion. As systems scale, automate, or cross language and organizational boundaries, this erosion accelerates while confidence paradoxically increases. The result is a class of systems that function correctly but cannot be reasoned about reliably. We conclude by identifying the category of a missing layer between intent and execution: a human-reviewable semantic authority that captures local meaning, constrains implementation, and enables continuity beyond individual expertise.
1. The Fragmentation of Intent
In most software systems, intent is not captured in a single place. It is distributed across tickets, design documents, code comments, informal conversations, and the unwritten understanding of individual contributors. This fragmentation is not accidental; it emerges naturally as systems grow and teams attempt to remain productive.
Tools such as UML and system-level modeling attempt to centralize meaning, but they tend to grow too large to remain navigable. As a result, they are consulted infrequently and eventually become historical artifacts rather than active semantic references.
At the level where execution actually happens—the individual function or component—intent is rarely fully articulated. What a function is for, what it assumes about its inputs, what it guarantees about its outputs, what error paths are meaningful, and what invariants must hold for the surrounding system to retain meaning are seldom captured explicitly in one place. Instead, this understanding is reconstructed by each implementor from scattered sources.
This reconstruction works only because humans are capable of improvisation. Implementors synthesize partial information, make reasonable assumptions, and fill gaps silently. The system functions, but its meaning is no longer fully readable anywhere.
2. Why Existing Tools Do Not Anchor Meaning
When teams recognize this problem, they reach for familiar tools. These tools help, but they operate under assumptions that do not hold in practice.
Tests validate behavior against the most recent local understanding of a function. They confirm that an implementation behaves as expected today, not that it preserves the original or intended meaning over time. System tests exist, but they mirror the same fragmentation as system intent itself: they evolve independently, are owned by different teams, and introduce yet another distributed semantic surface. Passing tests often means alignment with the last known agreement, not with enduring intent.
Specifications attempt to describe intent, but they assume faithful and comprehensive consumption by implementors. In reality, no single contributor can hold the entire system in mind, even when documentation exists. Specifications describe the whole, while implementations live in parts. Each implementation would need the portion of the specification that normatively applies to it, yet such localized semantic authority is rarely present.
Type systems constrain shape and structure, but they do not encode obligation, purpose, or meaning. They prevent certain classes of errors without expressing what must remain true for the system to make sense.
LLM prompts introduce more dangerous assumptions. They presume that intent has been fully and unambiguously stated, and that the resulting output will be deterministic or close enough to the original intent. In practice, prompts are incomplete, models are nondeterministic, and the gap between stated and intended meaning is bridged by inference.
Code reviews assume that reviewers understand intent. When intent is not explicit and local, reviews devolve into checks of implementation detail rather than validation of semantic preservation. Reviewer and implementor alike operate under the same uncertainty.
Across all these tools, the shared assumption is that meaning is already stable, shared, and accessible. It is not.
3. The Threshold Where This Becomes Dangerous
This problem becomes dangerous surprisingly early. A system need only exceed a handful of components before no single person can reliably hold its full intent.
Once intent is distributed across people and artifacts, continuity depends on human presence rather than system structure. When a key individual leaves, intent does not degrade gracefully—it disappears abruptly. The system continues to operate, but without a clear understanding of why it exists or what it is meant to preserve.
Time compounds the problem. New contributors arrive, documentation lags reality, and onboarding becomes an exercise in reconstructing meaning from behavior. As iteration accelerates, each local change slightly reinterprets intent. Over time, the system drifts further from its original purpose in a manner analogous to a semantic telephone game.
Cross-language systems amplify this effect. When implementations span languages, intent is often carried primarily through comments and conventions. Semantic translation occurs implicitly, without a shared reference, much like communication between human languages without a dictionary.
AI-assisted generation introduces a qualitative shift. Generation assumes the prompt was followed. Review assumes the prompt and the output encode the same intent. When generation and test creation are both automated, systems begin validating their own interpretations of intent. Tests pass, confidence increases, and missing intent remains unexamined. Automation multiplies the danger while masking it.
4. The Missing Layer Between Intent and Execution
What is missing is not another tool, framework, or process. It is a category.
There must exist a human-reviewable artifact that captures intent locally and normatively, and that constrains implementation rather than trailing it. Verification must be anchored to this declared intent, not inferred from implementation behavior.
Every piece of a system has a role. That role carries meaning, and that meaning is intent. Intent must therefore be a first-class citizen, scoped to the unit that realizes it, and readable without system-wide context.
This intent should be executable in a human sense: an implementor should be able to read it, fully understand obligations and constraints, and produce a realization without guessing. It should sit between ideation and code—more precise than high-level specifications, but independent of any particular implementation.
Existing specifications gesture in this direction, but they are typically too global, too abstract, and too cognitively expensive to function as local semantic authority. What is needed is a compact, localized semantic contract—something closer to a humane DNA than a system blueprint.
5. What Accumulates When Meaning Is Ignored
When systems continue without such a layer, they accumulate something worse than technical debt.
They become systems that operate but cannot be explained. Meaning becomes baggage that must be passed orally, not knowledge that can be transferred. Reasoning about the system increasingly requires specialists—often the original authors—who carry intent in their heads.
Onboarding grows harder, change becomes riskier, and failures emerge that resemble normal operation. The system functions, but its purpose is opaque. Confidence persists, even as understanding fades.
These systems do not fail loudly. They fail quietly, by becoming meaningless while still running.

This paper does not propose a solution. It names a structural gap that becomes unavoidable as systems scale, automate, and delegate execution to machines. Until intent is captured explicitly, locally, and normatively, software systems will continue to succeed operationally while failing semantically.
